The Kali Gandaki runs between Dhaulagiri and Annapurna with seven vertical kilometres of mountain on either side. That makes it the deepest gorge in the world, and it puts everything north of it in a rain shadow. Walk up it and the country changes completely. There's subtropical forest and waterfalls at Tatopani, apple orchards and pine at Marpha, and by the time you reach Muktinath at 3,760 m it's bare ochre cliffs, prayer walls and Tibetan-Buddhist villages. The temple complex at Muktinath is sacred to Hindus and Buddhists alike, with 108 water spouts and a natural gas flame burning over a spring. Because the rain falls on the south side, this is the trek we run in July and August, when the trails further south are wet and full of leeches. On monsoon departures we add a spare day in case cloud stops the Jomsom flight.

No. Plenty of trekkers pass through on the Circuit or the Jomsom route. Behave as you would at any active place of worship and you'll be welcome.
It can be. Muktinath's 3,760 m is high enough to matter if you fly or drive straight up from Pokhara at 800 m. Take it slowly when you arrive, drink plenty, and don't plan anything strenuous for the first afternoon.
